Below are the available forms for filling out online or printing for the Bus Passes section.

Please make sure you have read the Bus Passes page first before filling in these forms!

What forms do I need to fill in here?

• If you are applying on the basis of your age complete the 'Bus Pass Application' Form
• If you are applying on the basis of a disability complete the 'Bus Pass Application Form' – AND if appropriate the relevant proof of disability form (available to download on this page below the guidance notes for disabled applications)
• If you are requesting a replacement bus pass or informing us of a change of details, complete the 'Replacement Card or Change of Details' form.

How do I return this form?

• If you are sending us a passport photo with your application form
  
  You can return it via post to: Bus Pass Application, Worthing Borough Council, Civic Centre, Ham Road, Shoreham-by-Sea, West Sussex, BN43 6PR.
  
  
• If you are not supplying a photo and need us to take one for your application
  
  You can come to the Town Hall, weekday morning's to have your photo taken digitally and we will take your application form there and then.

If you are applying on the basis of a disability, please read the guidance notes on acceptance for a disabled persons concessionary pass below.

Where you are unable to provide the proof of disability referred to in the guidance notes, please print off the following form for the appropriate disability which will need to be completed by you and your consultant or specialist.

If due to your disability you are unable to travel without the aid of a companion, the following form will need to be completed by you and your Doctor / Specialist:
